Staff was incredible and took good care of my mom. The administrator, HR and social services were excellent and very responsive to our concerns. Thank you for all that you did.

This place gave me all the care and support I needed to get back on my feet. The staff was very helpful and the physical therapy was phenomenal. Can’t say enough good things about the physical therapy staff. Highly recommend

Brought my grandpa here for therapy after he said he wanted to try it rather than go home on hospice. Unfortunately he declined quickly so we moved him home on hospice but the facility worked with hospice to make it happen on short notice over the weekend. The facility met expectations, we as his family tried to be there as much as possible, especially during meals to help feed him which I would encourage everyone to do if you have a loved one in a nursing facility. Too many people dump their family in these places and only show up to complain about their care occasionly.

Therapy is great. Nursing and the assistants are also really helpful. The food is good too.

We had our mama here for a short stay after her surgery, and all in all, it was a good experience. The therapy folks were real kind and worked hard to get her back on her feet. Most of the nurses were attentive and helpful, though there were a couple shifts where things felt a little slow. Her room was clean, and the place didn’t have that strong nursing home smell like you sometimes expect. The food was alright. Some meals she enjoyed, other times she just picked at it, but they were good about offering something else if she didn’t want what was on the tray. The building’s a little older, but it feels homey. It wasn’t perfect, but they sure did care. We’d definitely keep this place in mind if any of us need care again down the road.

Zero stars wasn’t an option, so I had to give Hillsboro Post Acute Care one star to write a review. DO NOT place your loved ones here . The place is dirty, cramped, in poor repair, and understaffed with nurses who refuse to answer any questions or concerns. Since being transferred to long term care, they have moved my mom four times, each time to a more and more cramped room , this time I found all of Mom’s personal belongings such as family photos and special gifts from loved ones, things that make her room seem “home like” in a pile on her shared closet floor. While visiting Mom last week, a nurses aid assisted mom’s roommate to the toilet, where he proceeded to pull the roommates pants down, and remove her depends to set her on the toilet with the bathroom door open, three or four feet from where my brother and I were setting, in full view of the now half naked roommate, after asking the nurses aid to “please close the door”, he finally did. This place clearly has no respect for the dignity, modesty and privacy of it’s residents. On the same visit, mom had been in her room three or four days, I noticed that my bedridden mother’s physician ordered air mattress was not plugged into the wall properly and was not inflated. So, either they did not plug it in after moving her into the room, or her roommate unplugged it, meaning that this crucial, life supporting equipment is not being monitored properly. My mom was not at all incontinent, and walking and living independently in October of 2025 when she entered this facility for physical therapy. Within a month she was no longer able to stand or walk because she is kept sedated so she doesn’t call out when her needs aren’t being met within reasonable amount of time. Within two months of being in this facility my mother has an order to have a permanent catheter. She was care flighted to UC Hospital after picking up a urinary tract infection with a drug resistant bacteria at this facility. The ER doctor asked me when the last time the catheter had been changed because it was the “worst he had ever seen”. In the photos I posted a photo of how nasty mom’s catheter tube looks today. All this place cares about is getting your loved ones money. Inmates in prison are living much better than the residents at this facility do. Never leave anyone you care about here. Update: Legacy Nursing Home in Hillsboro has changed their name to Hillsboro Post Acute, but nothing has changed except for the name. In March of 2025 I found my mom in severe distress and pain, begging me to call an ambulance to take her to the hospital to be evaluated, asking that I not let the nurses at the nursing home touch her. I asked the buses to help her anyway, and when the nurse picked mom’s catheter tubing up off of the top of mom’s thigh. The tubing peeled hunks of skin off. When I entered Mom’s room that day, I had to pick mom’s catheter bag up off of the floor and hang it on her bed, where it should be. The nurses told me that they weren’t sure why she was in such pain and crying, but they would go ahead and change mom’s catheter, and that mom had been “just fine” all day, when clearly she had been crying and in distress for a very long time, possibly hours. I decided it would be a good idea for Mom to be evaluated at Highland District Hospital, so I called an ambulance, as my asked me to do for her the moment she saw me. I was waiting outside when the ambulance showed up, and walked them to Mom’s room, explaining to the Paramedics what was going on with mom. When we got to the room the nurses would not let the Paramedics open the door, or see Mom to evaluate her. I walked outside to my brothers truck to tell him what was happening, when the nursing director called to say I would be arrested if I visit my mom again
